Creatine for healthy aging: useful, inexpensive, and often oversold

Creatine has better evidence than most supplements. Its strongest case is still strength and training. The claims about memory and brain health are more interesting than settled.

What creatine actually does

Creatine helps replenish phosphocreatine, a rapid source of energy used during short, demanding efforts. Your body makes creatine, and food provides some, especially meat and fish. A supplement increases the amount stored in muscle for many people.

That can support a little more work in repeated sets, sprints, or other brief high intensity efforts. Over time, doing slightly more quality work can contribute to strength and muscle gains. Creatine is not a stimulant, steroid, or substitute for training.

The dose for strength and muscle

The simple approach is 3 to 5 grams of creatine monohydrate every day. Timing is not important enough to complicate the routine. Take it with a meal, after training, or whenever you will remember it. The muscle stores build over several weeks.

A faster approach is a loading phase of about 20 grams a day, split into four 5 gram servings for five to seven days, followed by 3 to 5 grams daily. Loading works faster but can cause more stomach discomfort and a quicker rise in water weight. It is optional. There is no good reason for most people to cycle on and off.

Why it may matter more after 50

Resistance training remains the central intervention for preserving strength. Studies in older adults suggest that adding creatine can improve lean mass and some measures of strength more than training alone. The average effect is modest, and not everyone responds the same way.

This makes creatine a possible supporting tool, not the program. I would put money and attention into a safe resistance plan, adequate protein, sleep, and consistency before building a supplement shelf.

What we know about the brain

The brain also uses creatine. That biological logic has led to studies of memory, attention, processing speed, sleep deprivation, mood, and neurological conditions. Some trials and meta analyses report small improvements in memory or selected cognitive tasks. Others find no meaningful overall effect.

The inconsistency matters. Study populations, tests, diets, and doses vary. A 2024 European Food Safety Authority review concluded that a cause and effect relationship between creatine supplementation and improved cognitive function had not been established. For now, there is no validated brain dose.

If someone already chooses 3 to 5 grams daily for training, possible cognitive benefit can be viewed as an open question. I would not jump to 10 or 20 grams a day solely for brain health without clinical guidance and a specific reason.

Safety, water weight, and kidney tests

Creatine often increases body weight by drawing more water into muscle. That is not the same as gaining body fat. Mild stomach upset can occur, especially with large single doses. Splitting the dose or skipping a loading phase usually helps.

Research in healthy adults has generally found creatine safe at common doses. Creatinine, a blood marker used to estimate kidney function, can rise because creatine breaks down into creatinine. That can complicate interpretation without necessarily indicating kidney damage. Tell the clinician ordering your labs that you take creatine.

Existing kidney disease, pregnancy, complex medication use, or an unexplained change in kidney tests deserves an individualized discussion before starting.

A conservative way to try it
